Once you have completed the form above, our Office Manager, Shannon will reach out to you via text or phone call. During this conversation, Shannon will help you choose the right therapist, address any questions you may have, and email you the necessary paperwork to get started. These documents can be conveniently completed through our Client Portal using either a computer or your cell phone.
For Insurance Clients: If you plan to use insurance for our services, please ensure that you upload a copy of your Driver's License and Insurance card to our secure portal. Once all the required documents are completed, we will verify your insurance benefits, and notify you of your out of pocket costs. Then we will be ready to schedule your intake session, whether it be via telehealth or in person. Rest assured, you will receive a text message containing further instructions including our address and a photo of the building.
